Navigating the Kentucky BioTech Facility

The mission begins with Case and Marshall entering a Kentucky BioTech facility filled with toxic gas, necessitating gas masks. An elevator malfunction leads to a fall, gas mask breakage, and subsequent hallucinations. The player regains control after a cutscene.

Unlocking the First Door and Reaching the Security Desk

Locate a locked red-lit door. Use a hatchet found on a mannequin to pry it open. Proceed through the hallway, up the stairs, and to a central elevator. Activating the elevator triggers a zombie transformation of the mannequins. Eliminate them with the hatchet. A ringing phone at the circular desk provides instructions to locate four director's cards (Red, Green, Blue, Yellow). A map points to the Yellow card's location.

Acquiring the Yellow Card and Grappling Hook

Follow the map to a yellow staircase leading to a director's office. Solve the computer puzzle ("Access" and "Lift") to access the A.C.R. room. Eliminate additional zombies. A mannequin holding the Yellow Card transforms into an abomination upon approach. Before engaging, gather armor plates, weapons, and a crucial grappling hook from the surrounding area. Use tactical explosives (C4 or grenades) to efficiently eliminate the abomination and its zombie horde. Retrieve the Yellow Card.

Obtaining the Green Card

Use the grappling hook to ascend to the main facility. Locate the Administration Facility by grappling up from a point left of the security desk elevator. Answer the ringing phone. Find four documents and place them in the file display area. Maneuvering around the pursuing mannequins (they freeze when you turn) will help you locate the documents: one on a corner desk, one near a round table, one on a small central table, and one in the cafe. Defeat the resulting Mangler Zombie to obtain the Green Card.

Securing the Blue Card

Grapple across to the Joint Projects Facility. Answer the phone. Locate the glass chamber with camera stands, and find the Blue Card. Eliminate the spawning Mimic (it disappears; shoot moving objects to reveal it) to claim the Blue Card.

Retrieving the Red Card

Head to the East Wing (marked on the initial map), following red carpets to stairs and a room with water, a console, and a Mangler. Interact with the console to reveal the Red Card. Use the grappling hook to reach the upper area, swim through a red tunnel, climb ladders, and eliminate zombies. Unlock a door using a blacklight-revealed code, and turn all drain switches within 25 seconds to access the Red Card. Defeat the Mangler and its zombie horde.

Confronting the Disciple

Return to the security desk, insert all four cards, and take the elevator to the top floor. Eliminate any pursuing zombies. Answer the ringing red phone. Engage the horde of zombies and the Disciple in the BioTech room. A final cutscene reveals the encounter as a hallucination.
